Adding an advanced Contact us form to your Drupal based website with File Attachment feature is ver yeasy. There is a module called "Webform" that adds a webform nodetype to your Drupal site.
A webform can be a questionnaires, contact us or request form. These can be used by visitor to make contact or to enable a more complex survey that the type polls enable. Submissions from a webform are saved in a database table and can optionaly also be mailed to an e-mail address upon submission.
Follow the simple steps below to have a contact form that hasfile attachment capabaility.
Steps to create contact us form with file attachment in Drupal.
- Download and enable the WEBFORM module from http://drupal.org/project/webform
- Go to Administer > Create Content > Webform and create a webform by filling in the usual fields, you may want to assign a menu link as well.
- After the webform creation, you have to add form components, see the following screenshot. Add text or email fields and here you also have an option of adding "file type component. This is for enabling file attachments in contact form of" your Drupal based website. You can have multiple file attachments by having multiple file upload fields in your contact form.

After adding file component, you can select the maximum file size to allow for the attachment with your contact us formSave your form and thats it.

You have now a contact us form with file attachment functionallity for your Drupal based website.
Using this module, you can very easily create feedback form for Drupal, online query or inquiry form for your Drupal site, a simple order placement form for your Drupal based application or use it for your own custom requirements.

This only attaches a single file not multiple files
